insurance payments? 1. Patient Demographic Form / Patient Registration Form 2. Copy of the front and back of patient insurance card (one-time only) 3. Patient Superbills or Daysheet 4. Explanation of Benefits
Who
receives the insurance payments? Insurance payments and Explanation of Benefits are sent directly to the provider’s address.
How
long does it take to receive insurance payments? For electronic filing, most payments are received within 14-21 days. Paper filing takes approximately 45 days.
Do
we send originals of everything? According to legislation, you should keep originals and send us copies only, which we will retain for a term according to state medical record retention laws. |
